Elements Support Services - Intensive Assessment Service
Our individualized program is custom designed for each client. We work with the community treating Rehabilitation Team and implement a comprehensive assessment utilizing a community-based approach. During this time, we collect extensive data to support the treating team in completing an in-depth assessment of the client’s functional, psychosocial, behavioural and cognitive skills. We are an award-winning rehabilitation service that meets the needs of people with an acquired brain injury. Our program is intensive and incorporates the comprehensive assessment results into real, meaningful, and functional programs.

What Does This Include
In consultation with the Rehabilitation Team, develop, assess, design, and implement the overall treatment plan
Provide ongoing tracking, evaluating and monitoring of goals
Mentor/coach the client and their support network to ensure identified goals are achieved
Complete progress reports and forward to funding source and other identified professionals at regular intervals Schedule all daily activities
